Responsibilities
- Bring empathy and dedication to every in-home visit as part of a national network of clinicians
- Participate in nationwide efforts to visit millions of people and connect them to appropriate care
- Fulfill the requirements of the specified clinician role (Nurse Practitioner, Physician Assistant, or Physician)
- Conduct virtual visits as needed
- Provide all Signify Health services, including Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screening)
- Assess a person’s health and well-being in their home environment
- Perform medication and medical history reviews
- Conduct physical evaluations (approximately 45 minutes per visit)
- Answer health questions and build trusted relationships
- Utilize an iPad and intuitive clinical workflow for evaluations
