Requirements

  • Reside in Region 5: Kankakee, Will, or Grundy Counties
  • Travel: 70%
  • Schedule: 8-4:30pm CST
  • 1-2 years’ experience providing clinical services; experience in community/outpatient setting preferred
  • Community Health Worker Certification or equivalent training and experience preferred
  • 3-5 years relevant experience
  • Experience serving Medicaid populations in urban or rural environments with familiarity of local formal and informal resource networks preferred
  • Preferred training: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, specialized training in medical terminology and practices for designated health conditions
  • Meet all local state requirements (e.g., first aid, CPR, TB screen)
  • Experience using a computer including the use of Excel

Responsibilities

  • Conduct home visits within Region 5 IL (Kane County) with Aetna members with chronic conditions to reinforce care plan adherence, adjust tasks to fit member’s culture/language/religion, improve access to care, identify health needs, and reduce unnecessary hospitalizations
  • Educate healthcare professionals and providers on social determinants of health impacting members via various communication channels
  • Build and maintain positive working relationships with Aetna members, providers, nurse case managers, agency representatives, supervisors, and office staff from diverse cultural and socio-economic backgrounds; reduce cultural and socio-economic barriers
  • Assist with care plan implementation and member education during in-home visits, develop care management strategies, and provide linkages for health and social needs
  • Assist members to identify socio-economic issues affecting health and develop health/social management plans and goals
  • Identify gaps and opportunities to strengthen community systems of care; assist members in utilizing community services, including scheduling appointments with social services agencies/transportation vendors and completing applications for eligible programs
  • Assist with patient medication adherence by instructing on current medication list, reviewing medications, and helping obtain refills
  • Teach disease self-management (e.g., nutrition, symptom tracking and reporting)
  • Accompany members to appointments as needed
  • Enter and maintain member records in electronic health record system, compile reports, complete program documentation (e.g., progress notes, letters), and other administrative responsibilities in a timely manner
  • Travel extensively to client homes, community locations, various agencies, and other outreach destinations
  • Work under the direction of a designated professional
