Requirements

Candidates must possess a Bachelor's degree and at least 2 years of progressive experience in care coordination, community health, or health promotion. A valid driver's license and a personal vehicle for daily use are required. Proficiency in technology for member support, including scheduling and electronic health record systems, is necessary. Familiarity with Motivational Interviewing and Trauma Informed Care principles, along with strong problem-solving and adaptability skills, are essential.

Responsibilities

Community Health Partners will collaborate with RN Care Managers to support integrated social care coordination for members with complex needs. This involves conducting assessments, developing care plans, and assisting members in achieving their health goals. Responsibilities include engaging members, providing non-clinical education, facilitating referrals, and ensuring loop closure on internal referrals. CHPs will also maintain accurate records using care facilitation and EHR platforms, manage a high-volume caseload, and conduct community-based visits as needed.
