Candidates must be independently licensed mental health professionals, specifically LCSW, LMFT, or LPC, with experience supporting members with chronic and treatment-resistant mood and anxiety disorders. The role requires proficiency in active listening, psychoeducation, motivational interviewing, and Dialectical Behavioral Therapy (DBT) skills. Strong written communication and documentation skills, including EHR experience, are necessary, as is the ability to interact with multi-disciplinary teams. A Bachelor's degree is implied by the professional licensure requirement.
The Specialty Care Navigator will conduct telephonic screenings, provide brief interventions, and refer members to specialized treatment providers for mood and anxiety disorders. They will offer support and coaching, set goals with members, and coordinate care with Spring Health providers, identifying the appropriate level of care. Responsibilities include providing consultation on mental health and relationship issues, offering in-the-moment support and safety planning for members in crisis, and maintaining accurate records and documentation. The role also involves problem-solving barriers to care, collaborating with various internal teams, assisting other specialty care programs as needed, and participating in meetings and training. Additionally, the navigator will help onboard and mentor new team members, adhere to company policies, and maintain confidentiality and ethical standards.
