Responsibilities

Triaging incoming referrals for members with social needs
Providing operational and administrative support across the team
Working hand-in-hand with other Devoted teams to address member needs
Calling a subset of members directly to connect them to community services
Reaching out to local organizations to ensure members are receiving appropriate support
Completing necessary paperwork for enrollment in public and community programs
Using an internal resource library to address social needs
Meeting or exceeding performance and quality goals
Providing inbound call support for members who need Social Services Support
