Requirements

Candidates are required to have at least 2+ years of experience in behavioral health, social services, or a related field relevant to the program focus, proficiency in Microsoft Office Suite, and access to a private workspace. Preferred qualifications include case management and discharge planning experience, managed care experience, and a high school diploma or higher.

Responsibilities

The Care Management Coordinator will conduct annual Health Risk Surveys to identify member needs, escalate risks to the care manager, complete care coordination activities, inform care manager of quality issues, advocate for members, engage members through motivational interviewing, monitor and document care, meet performance metrics, demonstrate self-direction and a willingness to learn, navigate healthcare technology tools, and maintain professional relationships and effective communication skills.
