This role is a fully remote position focused on Transitions of Care (TOC) support for Devoted members. You will provide short-term interventions to members being connected to community-based behavioral health services, aiming to prevent avoidable emergency room/inpatient care and/or hospital readmissions. The TOC team is dedicated to providing culturally competent care with a focus on equity and quality, fostering a supportive team culture.

Requirements

  • Master’s degree in Social Work
  • Active clinical license in any US state (LCSW, LMSW, or comparable)
  • Minimum of 5 years of experience in the behavioral health or care management fields
  • Proficient in performing comprehensive psychosocial assessments via video and phone
  • Ability to independently manage a fully remote work schedule (M-F, 8:30 am - 5:00 pm local time)

Desired Skills and Experience

  • Bilingual in English and Spanish, Creole, or Vietnamese
  • Experience in Geriatric Behavioral Health
  • Experience in the health insurance industry (Medicare Advantage is a plus)

Responsibilities

  • Perform comprehensive psychosocial assessments via phone/video with members and caregivers.
  • Conduct clinical social work interventions, including:
    • Brief therapeutic interventions
    • Assistance in navigating family/support issues
    • Assessing and intervening on social determinants of health
    • Completing safety assessments
  • Collaborate with interdisciplinary team members (clinicians, nurses, care coordinators) to ensure cohesive, patient-centered care.
  • Clinically identify and escalate complex members to the BH Medical Director for consultation.
  • Actively participate in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development.
  • Document all clinical interactions in the electronic health record.

Compensation and Total Rewards

  • Salary Range: $55,000 - $79,000 / year. The actual base salary may vary based on qualifications, experience, skills, education, licenses, and location.
