Requirements

Candidates must hold a Master’s degree in Social Work with an active clinical license in any US state, such as LCSW, LMSW, or comparable state clinical license, and possess a minimum of 5 years of experience in the behavioral health or care management fields. They should be competent in performing comprehensive psychosocial assessments via video and phone, and able to independently manage a fully remote work schedule from Monday to Friday, 8:30 am to 5:00 pm local time. Bilingual skills in English and Spanish, Creole, or Vietnamese are desired, though not required. Experience in geriatric behavioral health or the health insurance industry, particularly with Medicare Advantage, is also preferred.

Responsibilities

As a Transitions of Care (TOC) Behavioral Health Social Worker, the individual will perform comprehensive psychosocial assessments via phone/video with members and caregivers, conduct clinical social work interventions to maximize program effectiveness, collaborate with interdisciplinary teams, identify and escalate complex members to the BH Medical Director for consultation, and document all clinical interactions in the electronic health record. They will also actively participate in team huddles, panel reviews, and program development activities.
