Care Coordinator

About the Role

Humana Gold Plus Integrated is seeking Care Coordinators to join our caring community. This full-time position focuses on assessing and evaluating member needs to promote optimal wellness. You will guide members and their families toward appropriate resources and facilitate interactions to support their care and well-being.

$2000 SIGN-ON BONUS for new associates! (Payable following 90 days of employment.)

Important: You must reside in the Barrington/Lake Zurich area in the State of Illinois (Lake/Cook County area).

Responsibilities

The LTSS Care Coordinator utilizes various strategies and techniques to manage member health issues. You will:

• Ensure members receive services in the least restrictive setting to achieve optimal well-being through needs assessments.
• Identify and resolve barriers that hinder effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services, monitor, and evaluate case management plans against member goals.
• Ensure patient progress towards desired outcomes through continuous monitoring via assessments, data, member conversations, and active care planning.
• Guide members/families toward appropriate care resources by facilitating interactions with payers, providers, interdisciplinary teams, and others involved in member care.
• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings.
• 75-90% local travel is required.

Required Qualifications

To be considered for this role, you must meet one of the following educational/licensure requirements:

• Bachelor’s degree in social sciences, social work, human services, or a related field.
• Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one (1) year of experience in comprehensive assessments and formal service provision for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Additionally, you must meet all of the following requirements:

• Must reside in the Barrington/Lake Zurich area in the State of Illinois (Lake/Cook County area).
• 1-year experience in healthcare and/or case management.
• 1-year experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations.
• Intermediate to advanced computer skills, including experience with Microsoft Word, Excel, and Outlook.
• Ability to use various electronic information applications/software programs, including electronic medical records.
• Exceptional communication and interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.

Additional Role Requirements

• This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. TB screening will be required if selected.
• This role is part of Humana’s Driver Safety Program. You must have a valid state driver’s license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
