Requirements

Candidates should have a background in behavioral health, such as a BS in psychology, social work, social services, or case management/coordination, or equivalent experience. Comfort working in a fast-paced, changing environment, self-starter and team player mentality, critical thinking skills, and a willingness to learn are essential. Experience or comfort with crisis management is required, and bilingual skills in Spanish or Chinese are a plus.

Responsibilities

Care Coordinators will serve as a communication channel between patients, providers, and the operational team, ensuring a seamless patient experience. Responsibilities include utilizing software and local resources for information, following up on no-show appointments, assisting with rescheduling, informing patients of provider absences, supporting discharge and re-engagement, facilitating internal referrals, and supporting patient engagement via phone calls and chat messages. They will also coordinate clinical huddles as necessary and perform other assigned duties.
