Requirements

Candidates must possess an active, unrestricted Louisiana Licensed Masters Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage Family Therapist (LMFT), Licensed Addiction Counselor (LAC), or an active, unrestricted Louisiana Registered Nurse (RN) license with at least three years of behavioral health experience. A minimum of two years of experience working as a behavioral health professional, experience with behavioral change, health promotion, coaching, and/or wellness, and proficiency with Microsoft Office Programs are also required. Candidates must reside in Louisiana and have a valid driver's license with reliable transportation, meeting specific vehicle liability insurance limits. Minimum internet download speed of 25 Mbps and upload speed of 10 Mbps is necessary for remote work.

Responsibilities

The Field Care Manager, Behavioral Health assesses and evaluates member needs and requirements to achieve optimal wellness, guiding members and families toward appropriate resources. They manage behavioral health, physical health, social determinant of health, and value-added benefits, employing various strategies to address member issues and remove barriers to effective care. Responsibilities include performing telephonic and face-to-face assessments, creating member care plans, monitoring progress, collaborating with providers and community services, coordinating across the care team, and submitting incident reports. The role involves up to 75% travel within assigned regions in Louisiana.
