Requirements
- Minimum one year of experience as a Verzorgende IG at Livio
- Contract of at least 20 hours per week (or willingness to temporarily increase)
- Openness to feedback and reflection on own actions
- Motivation to invest in professional growth and complete the required training
Responsibilities
- Coordinate (multidisciplinary) care and be responsible for the care plan (including intake, assessment, coordination, and evaluation)
- Participate in the MDO (Multidisciplinary Meeting) and identify when additional consultation is needed
- Signal bottlenecks and discuss them with your supervisor
- Support and coach colleagues in the implementation of care plans and provide functional instructions
- Serve as a point of contact and expert in your specialization area
- Provide information and advice to clients and their families
- Participate in approximately 20 training days spread over one calendar year (approximately 6 hours of study per day per lesson)
