Job Description
Summary
Identify, create, and implement home health clinical practice standards for in-home care management that deliver high value clinical and financial outcomes in support of our home health offering to patients, physicians, health systems, and payer partnerships.
Primary Focus
- Establish in-home clinical assessment and practice standards that align with in-home procedures and policies
- Establish protocols and standards for the in-home disciplinary care team to improve patient experience and mitigation of readmission
- Use evidence, train, and implement care standards in each HH region that drives favorable clinical and financial outcomes.
- Working closely with SVP-HH, develop reliable reporting on organizational metrics. This will include creating KPIs, clinical audit tools, productivity measures, and analyzing in-field work and impact on claims data
- Prepare and conduct clinical case reviews for the purpose of mitigation of hospitalization/ER/OBs use
Experience
- Minimum 5 years in progressive leadership and management roles in healthcare administration within a multi-site company are required.
- Substantial experience establishing standards and implementing programs and systems for clinical care and service is required.
- Demonstrated success at VP/Director level and ability to provide leadership and vision, develop partnerships, and influence all levels of organization
