Requirements

  • 2+ years of project management experience in a Healthcare Organization, Operations, or Technology function
  • Bachelor’s degree in Business, Supply Chain Management, Operations Management, Finance, or related field
  • Excellent problem solving abilities and a proactive approach to addressing operational challenges
  • Experience with project management and workflow tools like SmartSheets, Jira, Asana, or Lucid Charts
  • Project Management Professional (PMP) certification
  • Experience managing change and maintaining excellent relations with executives, customers, Operations and Product Management staff at all levels
  • Anticipated weekly hours of 40
  • Ability to work in a fast-paced environment and prioritize multiple projects simultaneously

Responsibilities

  • Overall program and project management of CCP for All
  • Build the pipeline by working with CS and Sales Ops to communicate with clients, get them to sign up, and manage the waves of implementation
  • Lead new CCP for All Implementation projects, including:
  • a. 40 new clients to implement in the remainder of 2025
  • b. Communicating with the Client Success team and Client to ensure clarity of roles, responsibilities, and expectations
  • c. Building project plans for each new client and driving project progress
  • d. Client contracting approval
  • e. Client scripting approval
  • f. Gathering client-specific pathway information and design decisions
  • g. Working with Engineering teams for technical pieces
  • h. Working with Salesforce engineer to add client-specific data (member services number/find a pcp url)
  • i. Working with each client and reporting team to set up and deliver standard reporting
  • j. Communicating with care coordination agents updates on live implementations
  • k. Determining and communicating to managers when volume dictates additional agent resources are needed
  • Identify and work to resolve issues that would delay the timeline of each project implementation
  • Complete data audits on new clients to ensure Signify and Client reporting are accurate
  • Update Care Coordination Reporting with weekly/monthly/quarterly metrics
  • Assist with other team projects, including multi-client rollout plans (e.g. Pre-Visit Questionnaire, Referrals)
