Requirements

Skilled nurse practitioner with thorough understanding of chronic disease management for common conditions like diabetes, hypertension, and hyperlipidemia
Thorough understanding of typical treatment modalities, medication management, and lifestyle modification

Responsibilities

Manage a refill queue for patients enrolled in primary care service and Medications on Time services
Perform virtual visits for members requiring additional medical evaluation to safely renew their medications
Accurately diagnose and manage chronic conditions, identifying and evaluating risk factors, comorbidities, and possible contraindications for treatment
Provide counseling on medication adherence and potential side effects
Collaborate with other care team members, including PCPs, endocrinologists, cardiologists, and pharmacy, clinical nursing, and social work teams
Interface with family members and caregivers to coordinate holistic care and deliver a collaborative care plan
Utilize the homegrown electronic health information system for visits and provide feedback on how to improve the interface
Maintain accurate and up-to-date patient medical records
Participate in quality improvement initiatives and ongoing professional development to stay current on best practices and advancements in cardiometabolic care
