Requirements

Nurse practitioner (APRN) with thorough understanding of chronic disease management for conditions like diabetes, hypertension, and hyperlipidemia, including treatment modalities, medication management, and lifestyle modification
Comfortable working in an environment with rapid change, continuous process improvement, and learning/using new technologies
Experienced in interprofessional/team-based care

Responsibilities

Manage refill queue for patients in primary care and Medications on Time services
Perform virtual visits for members requiring additional medical evaluation for safe medication renewal
Accurately diagnose and manage chronic conditions; identify/evaluate risk factors, comorbidities, and contraindications
Provide counseling on medication adherence and potential side effects
Collaborate with care team (PCPs, endocrinologists, cardiologists, pharmacy, nursing, social work), family, and caregivers for holistic care and continuity
Utilize electronic health information system for visits and provide feedback for improvements
Maintain accurate, up-to-date patient records in compliance with legal/ethical guidelines
Participate in quality improvement initiatives and ongoing professional development in cardiometabolic care
Adhere to laws, regulations, and standards (patient privacy, telehealth)
