Responsibilities

  • Manage refill queue for patients in primary care and Medications on Time services
  • Perform virtual visits for members requiring additional medical evaluation for safe medication renewal
  • Accurately diagnose and manage chronic conditions; identify/evaluate risk factors, comorbidities, and contraindications
  • Provide counseling on medication adherence and potential side effects
  • Collaborate with care team (PCPs, endocrinologists, cardiologists, pharmacy, nursing, social work), family, and caregivers for holistic care and continuity
  • Utilize electronic health information system for visits and provide feedback for improvements
  • Maintain accurate, up-to-date patient records in compliance with legal/ethical guidelines
  • Participate in quality improvement initiatives and ongoing professional development in cardiometabolic care
  • Adhere to laws, regulations, and standards (patient privacy, telehealth)
