The role is remote with 10% travel for offsite meetings, training, and educational purposes. It follows an 8:00am-5:00pm Monday-Friday schedule.
Candidates need a Masters’ degree with major coursework in social work, community/social services, and/or case management, minimum 1 year of experience with pediatric population, and minimum three years of postgraduate experience in health care social work/case management. License/certification such as LMSW, CCMC, or C-SWCM is preferred.

You will work alongside an interdisciplinary team including physicians, RNs, care team assistants, mental health therapists, community health navigators, pharmacists, dietitians, and more, using an integrated technology platform.
A strong candidate is passionate about reimagining pediatric health care for children with special needs, holds a relevant Masters’ degree, has at least 1 year pediatric experience and 3 years in health care social work/case management, and preferably has certifications like LMSW or CCMC.
