Requirements

  • Masters’ degree with major course work in social work, community/social services, and/or case management
  • Minimum 1 year of experience working with pediatric population required
  • Minimum three (3) years of postgraduate experience in health care social work/case management required
  • License/certification preferred (LMSW, CCMC, C-SWCM)
  • Knowledge and application of motivational interviewing practices and/or techniques
  • Comfortability working with acute populations and familiarity with school and community resources highly preferred
  • Experience with providing telehealth services and engaging with families in a remote environment
  • Bilingual Spanish strongly preferred

Responsibilities

  • Conduct thorough assessments, exploring the current medical, behavioral, and complex social needs
  • Identify barriers to care, such as food insecurity, unstable housing, transportation challenges, and insurance limitations
  • Collaborate with families and multidisciplinary teams to establish individualized care goals
  • Develop and maintain an integrated care plan addressing medical, behavioral, and social needs
  • Ensure timely follow-up on referrals, evaluations, and treatment recommendations
  • Offer education to families about navigating local resources and agencies
  • Provide emotional support and guidance when families are facing challenging situations
  • Advocate for resources and removal of barriers and connect families with available resources
  • Build and maintain relationships with community organizations that serve children with medical and behavioral health complexities
  • Stay informed on emerging programs and resources relevant to the population served
  • Represent the organization at community events, health fairs, and resource meetings
  • Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress
  • Perform other duties as assigned
