Registered Nurse (RN) licensure
Entry-level designation into the Clinical Advancement Program (CAP 1)
Ability to perform under the direction of the Supervisor with guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse
Complete CAP requirements to advance
Capability to handle physical demands including heavy lifting and moving of patients
Work in clinic setting and conduct initial home visits with another staff member present

Responsibilities

Empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment
Function as an onsite Patient Educator and Case Manager for patients
Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
Assess and manage patients’ response to home dialysis training and treatment therapy by following prescribed protocols and communicate issues to the physician
Participate in decision-making, teaching, leadership functions, and quality improvement activities as a member of the ESKD health care team
Perform ongoing, systematic collection and analysis of dialysis data, document in patient medical record, adjust treatment plans, and notify Supervisor or physician as needed
Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
Direct and provide, in collaboration with patient, home care partner, and staff, safe and effective delivery of dialysis therapy
Administer medications as prescribed or per approved algorithms, and document justification and effectiveness
Initiate or assist with emergency response measures
Serve as a resource for health care team, participate in staff training and orientation of new staff
Ensure correct laboratory collection, processing, shipping, and reschedule missed collections
Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with the Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and ability to perform treatments
Train Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of equipment and supplies through organized training program
Perform all other duties as assigned by Supervisor
