Requirements

Registered Nurse (RN) designation, meeting RN CAP 2 criteria or advanced from RN CAP 1
Ability to perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment
Capability to assess, plan, intervene, and evaluate clinically competent care for assigned patients
Skills in delegation of tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Knowledge of hemodialysis therapy delivery, medication administration per prescriptions or algorithms, and emergency response
Ability to complete CAP requirements to maintain or advance
Physical ability for heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, and moving equipment up to 200 lbs. (with assistance) or lifting solutions up to 30 lbs. as high as 5 feet
Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations

Responsibilities

Perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, adjust treatment plans, and notify supervisors/physicians as needed
Assess, collaborate, and document patient/family learning needs; provide initial and ongoing education
Direct and provide, in collaboration with staff, safe and effective daily hemodialysis therapy to assigned patients
Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
Initiate or assist with emergency response measures
Serve as resource, leader, coach, mentor, and role model for employees; set examples of behavior, work habits, and attitudes
Ensure correct laboratory collection, processing, shipping; reschedule missed/insufficient collections
Identify expected outcomes, document, and update nursing assessment and plan of care via Interdisciplinary Team collaboration
Ensure patient awareness of transplant and treatment modality options
Participate in education and quality improvement projects at facility and area level
Serve as Preceptor to new employees as needed
Perform all other duties as assigned by Supervisor
