Candidates must possess an Associate degree from an accredited School of Nursing and hold a valid Registered Nurse license in good standing. A minimum of two years of experience as a Registered Nurse, including specific hospice experience, is required. CPR certification is mandatory, along with the ability to exercise sound clinical judgment, document accurately, and demonstrate empathy, tact, responsibility, and organization. Understanding and participating in a team approach is also essential.
The Hospice Triage Registered Nurse will organize and direct hospice care, utilizing community resources to meet patient and family needs. Key duties include managing all triage calls during assigned business hours, responding courteously to patient and family inquiries, and making patient care decisions based on databases and nursing judgment. This role involves triaging after-hours calls to resolve issues, ensure patient safety, and prevent unnecessary ER/hospital admissions, as well as coordinating after-hours care, referrals, and staffing. The nurse will also review hospice referrals, assign staff, coordinate and communicate changes to all disciplines, and document patient encounters in real-time. Responsibilities include coordinating and assigning medically necessary visits, complying with company values, and performing other assigned duties.
