Candidates must hold an Associate's or Bachelor's degree in Nursing (BSN preferred), possess an active RN license in NY State (or compact licensure), and have a minimum of 5 years of nursing experience in ambulatory or outpatient chronic disease management, care coordination, or case management. Preferred certifications include Case Management certification (CCM), and strong clinical knowledge of chronic conditions such as diabetes, hypertension, COPD, heart disease, and others is required, along with demonstrated clinical judgment and critical thinking skills.
Responsibilities
The Registered Nurse - Chronic Care Management (CCM RN) will provide telephonic and telehealth chronic care management services for patients with multiple chronic conditions, conducting comprehensive assessments and developing individualized care plans in collaboration with patients and their healthcare providers. They will educate patients on disease management, medication adherence, and lifestyle modifications, coordinate with physicians and specialists, monitor patient progress, and support patients in navigating healthcare resources while utilizing electronic health records and Population Health platform to maintain compliance.
