Requirements

Candidates must be a Registered Nurse (RN) with active, unrestricted licensure in their practicing state, and a willingness to obtain additional state licensures as needed, possessing an Associate Degree in Nursing (ADN) preferred with a Bachelor of Science in Nursing (BSN) being beneficial. Active Nurse Licensure Compact (NLC) status is required, and 2 to 5 years of Nephrology Nursing and/or Case Management experience is desired.

Responsibilities

As a bilingual RN Care Manager, the role involves collaborating with healthcare teams to develop individualized care plans, educating patients, and coordinating transitions to dialysis through telephonic communication using motivational interviewing techniques. Responsibilities include mitigating hospitalization and mortality risks, participating in quality improvement activities, evaluating program processes, and making recommendations for program improvement, as well as partnering with providers and dialysis clinics to ensure optimal patient care.
