Requirements

Candidates must be a licensed Nurse Practitioner (APRN) with a current and unrestricted license in the state of practice, and must be fluent in both English and Spanish. The ideal candidate will possess a strong clinical background with experience in managing patients with common readmission conditions such as heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections.

Responsibilities

This Nurse Practitioner will conduct virtual clinical care visits for patients in the 30-day post-discharge period, performing comprehensive post-discharge assessments, developing individualized care plans, and providing disease-specific education. They will also diagnose, manage, and monitor decompensating patients, complete medication reconciliations, place orders, and collaborate with interdisciplinary team members including nurse case managers, clinical guides, and clinical coordinators to enhance patient care and program culture.
