Care Management Coordinator

The ideal candidate should possess an Associate’s degree, preferred, and at least one year of experience in home health or a related customer service capacity. They should also have at least one year of general office experience, with one year in data entry or word processing, and proficiency in the Microsoft Office suite. Previous Home Health experience is preferred, along with maintaining a professional license, certificate, or registration as applicable.
The Patient Services Coordinator will compile the daily schedules of clinical staff, assign POD clinicians to patients under the direction of the Assistant Clinical Manager, assist in care coordination, serve as a liaison between field staff, patients/clients, and POD personnel, communicate with CFSS regarding order approvals and staffing needs, process returned visits, and perform other duties as requested by management, including weekend rotations. They will also work within the HCHB workflow structure and communicate any workflow issues to the Assistant Clinical Manager prior to the end of their shift.
