Responsibilities

Richten op diabetes-, COPD- en CVRM-zorg voor cliënten met verpleeghuisindicatie op revalidatie- of tijdelijke opnameafdeling
Werken zelfstandig en in nauwe samenwerking met Specialist Ouderengeneeskunde (SO), verpleegkundig specialist (VS), zorgteam, cliënt en vertegenwoordigers
Uitvoeren van huisbezoeken
Afstemmen met andere disciplines binnen het multidisciplinaire behandelteam waar nodig
