Requirements

Active Physical Therapist license in the state of practice upon hire
Visual acuity adequate to perform position duties
Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
Ability to work the specified hours (full-time, day shift)

Responsibilities

Evaluate assigned patients, gathering data from patient records, interviews, observation, and team members
Develop and implement individualized treatment programs for patients, review plan of care with patient, and instruct patient, family, caregivers in home programs or exercises
Use selected activities and/or tasks to promote and maintain health, achieve treatment goals, and modify interventions and strategies based on ongoing evaluation
Provide direct therapy services to patients using various functional settings and strategies, planning therapy within the patient’s physical, emotional, and intellectual capabilities, coordinated with rehabilitation, medical, and nursing care plans
Prepare documentation as required, such as evaluation results, individualized treatment plans, progress notes, and other reports
Communicate effectively with patient, family, physician, and interdisciplinary team members regarding patient’s status, progress, and needs; attend care conferences when applicable
Establish rapport and work cooperatively with interdisciplinary team members (including family/physician) throughout the patient’s intervention program
Enhance professional growth through participation in educational programs, current literature, in-service meetings, workshops, etc
Oversee physical therapy assistants, techs, and students in an efficient and effective manner ensuring safe patient care and smooth departmental operation
Maintain knowledge of proper use of equipment for patient care
Commit to daily application of Avera’s mission, vision, core values, and social principles; promote values of compassion, hospitality, and stewardship
