Requirements

  • Functions as part of the dialysis health care team under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and corporation, state, and federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to setup and operate hemodialysis machines, calculate and enter patient-specific programming (e.g., Ultrafiltration (UF) goal, treatment time, UF modeling)
  • Ability to perform vital signs, weights, vascular access evaluation, machine safety checks, and documentation
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvement, and patient satisfaction initiatives
  • Training in facility policies for cleaning blood spills, hemostasis, dressings, and supply management

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, administer heparin as allowed
  • Obtain pre- and post-treatment vital signs, weight, and vascular access evaluation
  • Initiate dialysis treatment per prescribed orders (blood flow QB, dialysate flow QD)
  • Monitor patients’ response to therapy, evaluate intradialytic problems, provide interventions as prescribed
  • Obtain vital signs prior to reinfusion, discontinue treatment per procedures, evaluate patient (standing/sitting BP), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre- and post-dialysis evaluations
  • Calculate and enter dialysis machine programming for UF goal, treatment time, UF modeling; ensure safety alarms (e.g., Narrow Venous Limits 160) are enabled
  • Record vital signs, treatment parameters, and information on Hemodialysis Treatment Sheet and progress notes with initial identification
  • Report significant information/changes in patient condition to RN or supervisor
  • Observe patient, conduct machine safety checks, document interventions for unusual status
  • Monitor vascular access during treatment (arterial/venous pressures), intervene as needed, document/report unusual findings
  • Obtain prescribed laboratory testing, prepare specimens
  • Clean blood spills immediately with disinfectant
  • Prepare, organize, and utilize supplies/equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Assist in training applicable direct patient care staff
