  • 5+ years of experience as a Palliative Care Clinician
  • 2+ years of experience leading front line clinical teams with accountability for team level outcomes

Responsibilities

  • Provide expert clinical care for the Palliative Care Program
  • Provide direct supervision of palliative care clinical staff (NPs, RNs, SWs)
  • Provide direct patient care via phone or video visits (scheduled and on-demand)
  • Participate in and/or lead patient panel reviews
  • Participate in weekly IDT (1h/wk)
  • Act as clinical champion, modeling clinical excellence (player-coach)
  • Model and support development/implementation of new workflows during rapid iteration cycles
  • Goal setting and coaching of direct reports to achieve business goals
  • Regular review of direct reports’ work, hold RNs/SWs accountable to performance, create performance improvement plans when needed
  • Perform regular clinical audits (QA) including note review and listening tours
  • Steward member-centric and open clinical culture
  • Recruitment, interviewing, hiring, and onboarding of new clinical staff
  • Participate in referral management, process improvement, and change management
