The Telephonic Outreach Specialist is responsible for virtually connecting with members eligible for Cityblock services and engaging them around their needs. This role initiates the intake process, acting as the member's initial point of contact and liaison to a Community Health Partner. The Outreach Specialist serves as the member's primary contact until a long-term relationship is established, providing support, troubleshooting, education, and a listening ear. This role operates in conjunction with and receives support from the member's care team.
