Position Overview

The Telephonic Outreach Specialist is responsible for virtually connecting with members eligible for Cityblock services and engaging them around their needs. This role initiates the intake process, acting as the member's initial point of contact and liaison to a Community Health Partner. The Outreach Specialist serves as the member's primary contact until a long-term relationship is established, providing support, troubleshooting, education, and a listening ear. This role operates in conjunction with and receives support from the member's care team.

Requirements

  • 1 year of experience in a similar role.
  • Candidates must be able to commute to the following regions:
    • Maryland: Prince George’s County, Montgomery County, Baltimore City, or Baltimore.
    • Virginia: Fairfax County, Loudoun County, Arlington, Alexandria City, Manassas Park City, Manassas City, Prince William County, Fairfax City, or Falls Church City.

Responsibilities

  • Provide telephonic/virtual outreach and engagement services for eligible members.
  • Utilize data from Commons to understand potential members and inform them about outreach processes.
  • Review pre-populated demographic information and accurately document information and emergency contacts within Commons.
  • Collect verbal and written consent from eligible members.
  • Initiate the intake process and facilitate handoffs to the care team.
  • Communicate urgent needs and other pertinent information to care teams.
  • Daily use of Commons for data collection, information organization, task tracking, performance monitoring, and communication with care teams, members, and family members.
  • Provide feedback to the product development team and potentially join a super-user group for onboarding and support.
