Requirements

Candidates should possess at least one year of experience in a similar role, demonstrating an ability to commute to specified regions in Maryland or Virginia, including Prince George’s County, Montgomery County, Baltimore City, or Baltimore, OR Fairfax County, Loudoun County, Arlington, Alexandria City, Manassas Park City, Manassas City, Prince William County, Fairfax City, or Falls Church City. Candidates must be able to accurately collect and document demographic information and emergency contacts within Commons, and provide verbal and written consent for eligible members.

Responsibilities

The Telephonic Outreach Specialist will provide telephonic/virtual outreach and engagement services for each eligible member, leveraging data from Commons to learn more about potential members and inform them about the outreach processes. They will initiate the intake process and handoffs to the care team, communicate urgent needs and pertinent information to care teams, and work daily within Commons to collect data, organize information, track tasks, monitor performance, and communicate with care team, members, and family members. Additionally, the Outreach Specialist will provide feedback to the product development team and, over time, become part of a super-user group to assist in onboarding and supporting others.
