Requirements

Candidates must possess a high-school level education or equivalency and either a Medical Assistant Certification or at least two years of clinical experience in an outpatient setting. Spanish fluency is preferred, and proficiency in Excel is required.

Responsibilities

The Care Team Assistant will provide clinical and clerical support in a virtual Value-Based Care environment. This includes accepting and returning phone calls from caregivers and community partners, pulling daily lists for TOCs, and working closely with Outreach RNs. Responsibilities also involve creating TOC hospital-specific lists, placing calls, creating template-based letters, reviewing schedules, inputting instructions into Care Hub, and setting up PCP meetings. The role may also involve explaining the Imagine program and enrolling caregivers or children.
