Requirements

Candidates must be an APRN with 3 or more years of outpatient clinical practice experience. An active and clear RN and APRN license is required in one of the specified states (PA, GA, MS, MO, IL, HI, KY, TN, NC, SC, OH, IN, AR, WA, or AZ). Active BLS certification is mandatory at the time of hire and must be maintained. Desired experience includes primary care, family medicine, geriatrics, palliative care, performing Medicare annual wellness visits or in-home comprehensive visits, and conducting visits via telehealth video platforms. An understanding of managed care, including STARS/HEDIS measures, coding clinical comorbidities, and identifying care gaps, is a plus.

Responsibilities

The Nurse Practitioner will conduct primarily telehealth video visits to members, with the potential for a small volume of home-based visits. Responsibilities include performing comprehensive assessment visits (CAVs) that involve diagnosis/disease review, medication review, and assessment for quality of care interventions, as well as social and home health/DME needs. The role involves close collaboration with the member's care team, including PCPs, specialists, pharmacy, clinical nursing, and social work, as well as interfacing with family members and caregivers to coordinate care and develop collaborative care plans.
