Requirements

Candidates must possess a Master of Science in Nursing, an active National Nurse Practitioner certification, and a current, active Nurse Practitioner license in the state of Ohio, along with an active RN Compact license.

Responsibilities

The Nurse Practitioner will collaborate with physicians and providers to develop complex care plans, provide urgent healthcare via telephone and telehealth modalities to patients receiving home and community-based services, gather patient histories and conduct physical exams, develop and implement treatment plans, identify and close gaps in care, educate patients and families on various health topics, maintain patient medical records, and participate in continuing education while adhering to state regulations and company standards and policy.
