Thirty Madison is seeking a patient-centered and quality-focused Advanced Practice Provider to join Keeps Medical Group (KMG). This role involves caring for patients on our telemedicine platform, focusing on delivering quality medical expertise through virtual care. You will practice in an enriching environment dedicated to evidence-based medicine with advanced decision-support tools and clinical support. Compensation, Perks, and Benefits

• Base Pay: $96,880 to $133,210 per year (may vary based on knowledge, skills, experience, and licensure)

Responsibilities

• Consult and treat patients experiencing a variety of conditions.
• Proactively check in with patients on their treatment plans.
• Collaborate with customer service for operational support as needed.
• Respond to patient messages and adjust treatment plans accordingly.
• Provide feedback to improve quality of care and operational processes.
• Other duties as assigned.

Requirements

• Active NP/PA license.
• At least two (2) years of telehealth experience.
• Multiple state licenses required; must have at least 10 licenses.
• Must be able to work at least 16 weekend hours a month.
• Must be able to work 4 holidays and holiday weekends annually.
• Excellent written and verbal communication.
• No restrictions to be credentialed with Medicaid or other payers.
• Able to deliver empathetic, high-quality care to patients.
• Passionate about patient education.
• Willingness to work across various conditions/service lines.
• Comfortable working in a fast-paced environment.
• Fluency in Spanish (both written and spoken) is a plus.
