Requirements
Candidates must possess an active Nurse Practitioner (NP) or Physician Assistant (PA) license, with at least two years of telehealth experience and multiple state licenses, including at least 10 licenses. Applicants should be comfortable working at least 16 weekend hours per month and 4 holidays and holiday weekends annually, demonstrating the ability to deliver empathetic, high-quality care to patients. Fluency in Spanish (both written and spoken) is a plus, and there should be no restrictions to be credentialed with Medicaid or other payers.
Responsibilities
The Nurse Practitioner/Physician Assistant will consult and treat patients experiencing a variety of conditions, proactively check in with patients on their treatment plans, collaborate with customer service for operational support, respond to patient messages and adjust treatment plans accordingly, and provide feedback to improve quality of care and operational processes. They will also be responsible for other duties as assigned, focusing on delivering virtual care and supporting the Thirty Madison mission of providing access to healthcare for all who suffer from chronic conditions.
