Requirements

Candidates must possess at least 43 years of clinical practical experience and 243 years of care management, discharge planning, or home health care coordination experience. A valid and unrestricted Registered Nurse (RN) license in Colorado is required, or a compact RN license must be held. An Associate’s Degree in Nursing is required, with a Bachelor’s Degree preferred.

Responsibilities

The Case Manager, Registered Nurse will develop a proactive plan of care to address identified issues and enhance member wellness, utilizing clinical tools and information to evaluate member needs and benefits. They will conduct assessments considering various sources, such as claims, to address complex health and social indicators, collaborate with supervisors and stakeholders, present cases at interdisciplinary case conferences, and utilize motivational interviewing skills to engage members and discern their health status. Additionally, the role involves coordinating all case management activities, facilitating member wellness, and ensuring compliance with regulatory and company policies and procedures.
