Requirements

Candidates must hold a Master of Social Work (MSW) degree and possess a valid social work license in the state of practice. They should have a foundational understanding of Social Work Theory of Human Behavior and accepted methods of social work practice, along with experience working with patients undergoing dialysis and/or transplant processes.

Responsibilities

The Master Social Worker will conduct psychosocial assessments of patients receiving dialysis and/or considering transplant, collaborating with the interdisciplinary team to develop comprehensive care plans. They will provide supportive counseling services, educate patients and families, and facilitate access to community resources. The role involves monitoring patient adjustment to treatment, assisting with advance directives, and actively participating in process improvement activities to enhance patient quality of life and achieve FKC Quality Goals.
