As a Senior Program Manager within Interwell Health's Provider Operations Team, you will lead a team of Program Managers. The team acts as a liaison between the provider network and nephrology practices, facilitating a smooth transition to Value-Based Care. This role requires leadership skills, patience and adaptability, a diligent work ethic, excellent communication skills, and the ability to work independently and collaboratively with external partners and internal, cross-functional teams. The candidate should be a self-starter with a passion for the field and willing to grow within their career.

Responsibilities

  • Act as the Subject Matter Expert (SME) of intricate Value-Based Care Programs and/or agreements, including:
    • CMS Kidney Care Contracting
    • IWH Payor Agreements
    • IWH Practice Participation Agreements
  • Effectively communicate complex information in a clear, and concise manner to both internal and external stakeholders through meetings and presentations.
  • Provide outstanding customer service to all external and internal stakeholders with consistent feedback and follow-through.
  • Investigate and resolve any issues or concerns in a prompt and effective manner.
  • Assist our nephrology partners in achieving success within value-based care by uncovering valuable insights within data and identifying opportunities and strategies for improvement.
  • Execute company initiatives, ensuring the timely completion of deliverables.
  • Maintain accurate documentation of provider interactions/visits is maintained in Sales Force or, as directed by IWH, in any other designated tool.
  • Ensure your team delivers timely training and/or onboarding to Network Providers as new information is available.
  • Support and mentor Program Managers to maximize growth and team performance.

Requirements

  • Education: Bachelor’s degree required; Advanced Degree desirable
  • Experience: Experience leading teams to successful outcomes
  • Industry Experience: Experience working with physicians and practices, nephrology-specific experience a plus
  • Travel: Must be willing to travel up to 50%
  • Skills:
    • Superior attention to detail and exceptional follow-through
    • Independent and proactive approach, showcasing dedication to continuous learning
    • Excellent communication and interpersonal skills to effectively collaborate with providers, stakeholders, and internal teams
