As a Program Manager within the Provider Operations Team, you will serve as a crucial liaison between our provider network and nephrology practices. Your primary responsibility will be to facilitate a smooth transition to Value-Based Care. This role demands a diligent work ethic, excellent communication skills, and the ability to work both independently and collaboratively with external partners and internal, cross-functional teams.

Responsibilities

  • Act as a primary contact and subject matter expert for Interwell Health (IWH) Network practices.
  • Assist nephrology partners in achieving success within value-based care by uncovering valuable insights from data and identifying opportunities for improvement.
  • Execute company initiatives, ensuring the timely completion of deliverables.
  • Maintain accurate documentation of provider interactions and visits.
  • Deliver timely training and/or onboarding to Network Providers.
  • Verify that information presented to providers is both accurate and timely.
  • Bring a deep understanding of the objectives and complexities of intricate Value-Based Care Programs and/or agreements, such as:
    • CMS Kidney Care Contracting
    • IWH Payor Agreements
    • IWH Practice Participation Agreements

Requirements

  • Bachelor’s degree required; an Advanced Degree is desirable.
  • Proven experience working with physicians and practices; nephrology-specific experience is a plus.
  • Superior attention to detail and exceptional follow-through.
  • Must possess an independent and proactive approach, demonstrating dedication to continuous learning.
  • Excellent communication and interpersonal skills to effectively collaborate with providers, stakeholders, and internal teams.
  • Willingness to travel up to 50% of the time to practices (assigned regions will be discussed during the interview process).
