Requirements

Candidates should possess a Bachelor’s degree, with an advanced degree being desirable, and have proven experience working with physicians and practices, particularly with nephrology-specific experience being a plus. Superior attention to detail and exceptional follow-through are required, along with an independent and proactive approach demonstrating dedication to continuous learning, and excellent communication and interpersonal skills for collaborating with providers, stakeholders, and internal teams. Willingness to travel up to 50% of the time to practices is also necessary.

Responsibilities

As a Program Manager, the individual will act as a liaison between the provider network and nephrology practices, facilitating a smooth transition to Value-Based Care. They will serve as the primary contact and subject matter expert for IWH Network practices, assisting nephrology partners in achieving success within value-based care by uncovering valuable insights within data and identifying opportunities for improvement. This includes executing company initiatives, ensuring timely deliverables, maintaining accurate documentation of provider interactions, delivering training and onboarding to Network Providers, and verifying the accuracy and timeliness of information presented to providers.
