Requirements
Candidates must possess a Master of Social Work (MSW) degree and be licensed as a Clinical Social Worker (LCSW) or eligible for licensure. They should have a strong understanding of social work theory and accepted methods of social work practice, along with experience in psychosocial assessment and care planning.
Responsibilities
The Master Social Worker will conduct psychosocial assessments of patients undergoing dialysis, providing counseling and support to promote positive adjustment and improved quality of life. They will collaborate with the interdisciplinary team to develop care plans, educate patients and staff about the impact of Chronic Kidney Disease, and facilitate access to community resources. The role involves monitoring patient psychosocial status, providing supportive interventions, and participating in quality improvement activities related to patient satisfaction and quality of life, as well as assisting patients with advance directives and vocational rehabilitation.
