This is a field-based position requiring candidates to reside in Val Verde, Del Rio, Uvalde, Zavala, Ector, Tom Green, San Angelo, or Midland TX, with primary work in the field traveling to client sites and occasional office attendance for meetings or training.
Requires a current, unrestricted LPN/LVN, LSW, LCSW, LMSW or other license other than RN as allowed by state law, along with a Nursing Diploma or AS in Nursing or related field and minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
The role works under the direction and supervision of an RN, assisting with care coordination activities within the scope of licensure.
Strong candidates will have an MA/MS in Health/Nursing, along with the minimum required license, 2+ years of experience with chronic illnesses or disabilities in a service coordination role, and may need state-specified certification.
