Requirements

LPN/LVN, LSW, LCSW, LMSW, or license other than RN in accordance with applicable state law
Nursing Diploma, AS in Nursing, or related field
Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role (or equivalent combination of education and experience)
Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s)
May require state-specified certification based on state law and/or contract
Reside in Val Verde, Del Rio, Uvalde, Zavala, Ector, Tom Green, San Angelo, or Midland TX (field-based position)
Ability to travel to worksite and other locations as necessary

Responsibilities

Work under the direction/supervision of an RN, with overall responsibility for the member's case
Contribute to the LTSS care coordination process by performing activities within the scope of licensure, including assisting the responsible RN with telephonic or face-to-face assessments for identification, evaluation, coordination, and management of member's needs (physical health, behavioral health, social services, and long-term services and supports)
Assist responsible RN in identifying members for high risk complications
Obtain clinical data as directed by the responsible RN
Assist the responsible RN in identifying members that would benefit from an alternative level of care or other waiver programs
Provide all collected information to the responsible RN for verification, interpretation, additional assessments, and development/monitoring/evaluation/revision of the member's care plan
Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by responsible RN, in conjunction with the RN, member, and health care team, to ensure cost-effective and efficient utilization of health benefits
Apply decision-making skills based on current member needs, understanding of disease processes and terminology, and clinical guidelines (without requiring nursing judgment)
