Responsibilities

Conduct in-home health evaluations (approximately 45 minutes per visit)
Engage with patients one-on-one to answer health questions
Review medications and medical history
Perform a physical evaluation
Provide Diagnostic & Preventive Services (i.e., spirometry, DEE, bone density screening, etc.) as ordered
Collaborate with Signify Health as a clinician colleague
Conduct virtual visits as needed
