Requirements

  • MD/DO with active medical licenses in coverage area(s) (multi-state licenses preferred)
  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or similar format (preferred)
  • Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month
  • Execute a strong visit completion rate, consistently pacing 110+ completed visits per month
  • Execute a collaborating physician agreement, partnering with and overseeing Signify Health-contracted nurse practitioners or physician assistants (fulfilling all state-by-state requirements)
  • Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
  • Consistently engage with regular manager, team, and clinical meetings
  • Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
  • Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements; regular training required)
  • Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
  • Maintain all required supplies and associated diagnostic equipment related to DPS offerings
  • Be flexible to travel locally and within licensed states (may require hotel stay, car rental, and/or flight up to 20% annually)
  • Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies (including iPad, medical supplies, and scale)
  • Accommodate field observations and/or “ridealong” experiences, as requested

Responsibilities

  • Conduct pre-scheduled in-home visits with health plan members (Medicare Advantage, Medicaid, and certain commercial plan members), traveling primarily locally within a designated metro area (e.g., Fairfield, New Haven counties; majority of visits weekdays between 8 a.m. and 7 p.m.)
  • During approximately 45-minute visits, perform medication and medical history review, physical evaluation, and (if ordered by health plan) additional diagnostic tests (e.g., diabetic eye exams, spirometry)
  • Use pre-populated, patient-specific forms for each evaluation (no prescribing medicine, ordering lab tests, or altering treatment regimens required)
