Responsibilities

Conduct in-home health evaluations (approx. 45 minutes each), including medication and medical history review, physical evaluation, and answering health questions
Perform additional diagnostic & preventive services if ordered (e.g., diabetic eye exams, spirometry, bone density screening)
Use iPad and intuitive clinical workflow for evaluations (no prescribing medicine, ordering lab tests, or altering treatment regimens required)
Conduct virtual visits as needed
Provide all Signify Health services as indicated
Build trusted relationships, engage one-on-one, and connect patients to appropriate care to improve health outcomes
