Responsibilities

Participate in efforts to visit millions of people nationwide, providing connections to the right care
Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
Be flexible to travel locally and within licensed states, as needed
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening, etc.)
Provide in-home health evaluations, including medication and medical history review, and physical evaluation
Perform one-on-one connections, answering health questions (approximately 45 minutes per visit)
Conduct additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the person’s health plan
