Board-certified nurse practitioner (FNP, AGNP certification preferred)
Maintain active state licenses in coverage areas (multi-state licenses preferred)
Maintain board certification
Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
Execute a strong visit completion rate, pacing 35+ completed visits per month
Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)
Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
Maintain all required supplies and associated diagnostic equipment related to DPS offerings
Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
Be flexible to travel locally and within licensed states, as needed (up to 25%)
Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale)
Accommodate field observations and/or “ridealong” experiences, as requested
Primary coverage areas include but are not limited to: Springfield MA

Responsibilities

Conduct pre-scheduled in-home visits with health plan members (Medicare Advantage, Medicaid, and certain commercial plan members), traveling primarily locally within a designated metro area
Have the option to provide availability Monday - Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m
Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
During In-Home Health Evaluations (and other associated follow-up visits), conduct a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.)
Use a pre-populated, patient-specific form for each evaluation (not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen)
