Candidates must be board-certified nurse practitioners (FNP, AGNP certification preferred), maintain active state licenses and multi-state licenses (preferred), and have experience with in-home health evaluations preferred. Physical ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds is required.
You must provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month, and execute a strong visit completion rate, pacing 35+ completed visits per month.
The role requires conducting pre-scheduled in-home visits locally within a designated metro area like Springfield MA, with flexibility to travel locally and within licensed states as needed (up to 25%), and conducting video visits as needed.
