  • Board-certified nurse practitioner (FNP, AGNP certification preferred)
  • Maintain active state licenses in coverage areas (multi-state licenses preferred)
  • Maintain board certification
  • Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
  • Execute a strong visit completion rate, pacing 35+ completed visits per month
  • Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
  • Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)
  • Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
  • Maintain all required supplies and associated diagnostic equipment related to DPS offerings
  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
  • Be flexible to travel locally and within licensed states, as needed (up to 25%)
  • Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale)
  • Accommodate field observations and/or “ridealong” experiences, as requested
  • Primary coverage areas include but are not limited to: Springfield MA

Responsibilities

  • Conduct pre-scheduled in-home visits with health plan members (Medicare Advantage, Medicaid, and certain commercial plan members), traveling primarily locally within a designated metro area
  • Have the option to provide availability Monday - Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m
  • Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
  • During In-Home Health Evaluations (and other associated follow-up visits), conduct a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.)
  • Use a pre-populated, patient-specific form for each evaluation (not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen)
