Requirements

Responsibilities

The Homeless Services Liaison will engage with homeless Medicaid beneficiaries, establish relationships with community partners, and create a housing resource guide. Responsibilities include conducting intake interviews, assessing needs, coordinating services like medical care and housing assistance, and advocating for beneficiaries within the Medicaid system. The liaison will also educate providers on the challenges faced by homeless individuals, assist members with action plans for housing sustainability, collect and analyze data, and provide input on policy and program development.
