The Homeless Services Liaison (Care Management Support Professional 1) plays a critical role in ensuring that Medicaid beneficiaries experiencing homelessness receive the necessary support and services. This position involves collaborating with community-based organizations, healthcare providers, and government agencies to address the unique needs of homeless individuals and connect them with appropriate resources.

The Homeless Services Liaison (Care Management Support Professional 1) will serve an important role within the Equitable Population Health Management (EPHM) team to support members experiencing homelessness or at risk of homelessness with in-person assistance. The Homeless Services Liaison (Care Management Support Professional 1) will work as part of the Community Engagement team and coordinate with Humana’s Care Managers and Care Extenders (Care Managers), Community Health Workers (CHWs), Housing Specialists, and Health-Related Social Needs (HRSN) Coordinators to achieve care management goals and specific initiatives related to members experiencing homelessness.

Outreach and Engagement

  • Identify and engage with homeless individuals who are eligible for Medicaid.
  • Establish relationships with shelters, outreach programs, and other community partners to facilitate referrals.
  • Creates and maintains a housing resource guide that provides information on available support from social service agencies and community programs.

Assessment and Coordination

  • Conducts intake interviews and comprehensive assessments of homeless beneficiaries’ physical, behavioral, and social needs.
  • Collaborates with case managers, social workers, and healthcare providers to develop individualized care plans.
  • Coordinates services such as medical care, mental health treatment, substance abuse counseling, and housing assistance.

Advocacy and Education

  • Advocates for homeless beneficiaries within the Medicaid system.
  • Educates healthcare providers and staff on the unique challenges faced by homeless individuals.
  • Promotes awareness of available Medicaid benefits and services.
  • Assists members in the development of a solution-focused, goal-oriented action plan that will promote permanent housing sustainability.

Data Collection and Reporting

  • Collects and analyzes data related to homeless beneficiaries’ utilization of Medicaid services.
  • Prepares reports for management and regulatory agencies.
  • Uses data to identify trends and areas for improvement.
  • Maintains accurate encounter logs and outcome reports for each member.

Policy and Program Development

  • Stay informed about Medicaid policies and regulations related to homelessness.
  • Provides input on program development and policy changes.
  • Collaborates with other liaisons and stakeholders to enhance services.
  • Establish a policy/resource for mediation and advocacy with landlords on our members behalf.

Required Qualifications

  • Must reside in the state of Michigan.
  • Bachelor’s degree in social work, public health, or a related field.
  • Two (2) years of professional experience working for a public agency or in related fields such as social services, community outreach, or case management.
  • Experience working with homeless populations and knowledge of state -specific community resources.
  • Demonstrated ability to constructively work with persons in crisis situations.
  • Strong decision-making skills regarding own work approach/priorities, and work assignments, standards and resources.
  • This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • Ability to travel in the state of Michigan to meet face to face with members and/or their families, community partners and care teams.
