Requirements

  • 1-3 years of healthcare experience
  • Ability to assist the interdisciplinary care team to collaboratively plan, prioritize, implement, and evaluate individual community members’ plans

Responsibilities

  • Understand the requirements and intent of the Highmark Commercial contract to collaborate with the interdisciplinary care team to improve health plan member outcomes and support Aledade’s population health initiatives
  • Conduct direct telephonic outreach to health plan members based on outreach initiatives
  • Complete social determinants of health screenings and facilitate access to community/health plan resources (e.g., food, housing, mobility, energy assistance, governmental programs) via closed-loop referral process
  • Engage health plan members in obtaining preventative screenings and closing care gaps
  • Coordinate and document comprehensive tracking of health plan members’ adherence to appointments, health screenings, care plan objectives, non-clinical assessments, encounters, service plans, and outcomes
  • Under direction of Senior Manager, Care Management and in collaboration with primary care provider, assist members in understanding care plans/instructions, developing healthier habits, proper ER use, and alternatives
  • Build and maintain positive collaborative relationships with health plan members, caregivers, providers, interdisciplinary team members, and health plan representatives to deploy health engagement strategies and interventions
