Requirements

Candidates must have at least 2 years of experience working as a Health Coach and hold a Health Coach certification. They must be available to work between 8:00 AM - 5:00 PM MST or 7:00 AM - 4:00 PM PST. An Associate's degree or equivalent combination of education and experience is required. Community health, public health, managed care, Medicare, or Medicaid experience are preferred qualifications. Bilingual skills and a Bachelor's degree are also preferred.

Responsibilities

The Health Coach will provide information, referral, and supportive care to Medicare members within the Aetna Healthy Aging Support Program. They will instruct group coaching classes on chronic condition management, nutrition, safety, benefits, and mindfulness. The coach will assist members in actively participating in their healthcare decisions, document all member interactions, and troubleshoot resource unavailability. They will follow program workflows, follow up with members on action plans, and determine the need for additional referrals.
