The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Location and Travel
- Must reside in Michigan with the ability to drive to Wayne or Macomb Counties.
- This position will be based from a home office and will travel 75% of the time, to an assigned area in Wayne or Macomb county, to conduct in home visits with Medicare/Medicaid members.
Position Responsibilities
The Field Care Manager Nurse 2 employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.
- Responsible for managing a caseload and completing assessments with members in their home or community-based setting, as well as telephonically.
- Provides clinical support and guidance.
- Collaborates with ICT participants to support members with medical complexity.
- Helps develop and coordinate care plans ensuring that patients receive appropriate services to manage their health needs effectively.
- Addresses barriers to health care and advocates for optimal member outcomes.
- Reviews, assesses, and completes medical attestations and clinical oversights.
- Ensures members are receiving services in the least restrictive setting to achieve and/or maintain optimal well-being by assessing their care needs.
- Develops and modifies Individual Care Plan and involves applicable members of the care team in care planning (Informal caregiver, coach, PCP, etc.).
- Focuses on supporting members and/or caregivers utilizing an interdisciplinary approach in accessing social, housing, educational and other services, regardless of funding sources to meet their needs.
- Primary point of contact for the Interdisciplinary Care Team (ICT) and shall be responsible for coordinating with the member, ICT participants, and outside resources to ensure the member’s needs are met.
Required Qualifications

- Licensed Bachelor's prepared Social Worker with a minimum of 1 year in-home experience: case/care management, hospice, home health or equivalent environment.
- Michigan State licensure in field of study.
- Experience working with the adult population, disease management.
- Knowledge of community health and social service agencies and additional community resources.
- Exceptional communication and interpersonal skills with the ability to quickly build rapport.
- Ability to work with minimal supervision within the role and scope.
- Ability to use a variety of electronic information applications/software programs including electronic medical records.
- Excellent keyboard and web navigation skills.
- Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
- Ability to work full-time (40 hours minimum) Mon-Fri.
- Ability to travel to member's residence within 30 to 40 miles.
Driving Requirements
- This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.
- Valid driver's license, car insurance, and access to an automobile.
Home Office Requirements
- Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work.
- Must have accessibility to high-speed DSL or Cable modem for a home office (Satellite internet service is NOT allowed for this role); and recommended speed for optimal performance from Humana At Home systems if 25Mx10M.
Additional Information
- This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening.
