Requirements

Candidates must possess an MS in Nursing or MPAS/MSPAS, along with a current unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in California. DEA registration or eligibility and CPR certification are required. Experience with home-based services, complex care cases, hospice, palliative care, bedside hospital, oncology, ER, or ICU is preferred, as is experience with Epic EMR and bilingual English/Spanish skills.

Responsibilities

The Field Advance Practice Palliative Care Provider will deliver palliative care services in patients' homes and virtually, developing and implementing clinical plans of care for adults with chronic and complex conditions. This includes gathering patient history, performing physical exams, developing treatment plans aligned with patient goals, and addressing physical and psychosocial needs. The provider will educate patients and families, coordinate care with other clinicians and physicians, and maintain accurate patient medical records using Epic EMR. They will also prescribe medication as permitted by state authority and participate in continuing education.
