Nurse Practitioner - Telemedicine (W2 - Swing Shift)

Candidates must hold active and in-good-standing RN and APRN licenses, along with a current BLS certification. They must be willing to work four 10-hour shifts per week, including one weekend shift per month, with working hours from 11am-9pm EST, and be able to obtain licensure in additional states within 90 days of hire. Experience in primary care, internal medicine, urgent care, emergency room, geriatrics, and telehealth platforms is desired, as is bilingual proficiency in Spanish/English and experience managing acute/chronic disease exacerbations.
The Nurse Practitioner will deliver acute care via telehealth, diagnosing complex medical conditions, ordering and interpreting diagnostic tests, and developing care plans. They will work collaboratively with internal care teams, including physicians, other APRNs, medical assistants, and social workers, to coordinate member care. Responsibilities include performing acute care visits, assessing for quality of care interventions, and utilizing the electronic health information system. In some areas, weekend on-call support for clinical nurses may be required.
