Nurse Practitioner - Primary Care Clinician Manager (PCCM)

Position Overview

The Nurse Practitioner - Primary Care Clinician Manager (PCCM) is a vital member of the Clinical Leadership team, responsible for overseeing clinical care, professional development, and practice administration. This role is crucial for enhancing patient care and quality, improving practice communication, and partnering with internal teams to strengthen provider recruitment, training, and development. The PCCM cultivates a learning and working environment that fosters continuous learning, professionalism, and exceptional clinical quality. Responsibilities may include occasional travel, conference presentations, business development calls, and participation in clinical initiatives or Quality Assurance/Quality Improvement projects. The PCCM will collaborate with VPC Leadership to manage the practice effectively.

Responsibilities

Leadership

  • Coach and mentor clinicians, fostering a safe and high-performing environment.
  • Manage individual performance in conjunction with the Medical Director and TAO, taking corrective action as necessary.
  • Conduct regular staff meetings to promote engagement and growth.
  • Drive operational improvements and clinician productivity.
  • Act as a strategic partner across the organization to support key initiatives.

Communication

  • Serve as a liaison between clinical staff and leadership teams.
  • Provide clear, timely updates and ensure effective two-way feedback mechanisms.
  • Assist in organizing content for huddles and meetings.

Clinical Quality (QA/QI)

  • Collaborate with the Clinical Quality Team on QA/QI initiatives and annual reviews.
  • Recommend improvements based on clinician feedback and patient satisfaction metrics.
  • Deliver feedback to promote guideline adherence and patient safety.

Clinical Care

  • Dedicate at least 50% of time to patient care, ensuring timely and professional documentation.
  • Provide care for CCM-enrolled patients and exemplify clinical excellence.

Peer Review

  • Support the peer review process, conduct chart audits, and provide necessary clinician feedback.

Other Administrative Tasks

  • Undertake administrative projects as required.
  • Occasional business travel may be necessary.

Qualifications and Requirements

  • 5+ years of primary care experience as a Nurse Practitioner with an assigned patient panel.
  • 2+ years of virtual care experience.
  • Current, unrestricted Nurse Practitioner (NP) license in good standing with your state’s board of nursing.
  • Active national board certification as an NP through AANP or ANCC.
  • Must not be currently excluded or sanctioned by any state or federal healthcare program (including Medicare or Medicaid) and not listed as an excluded individual by the Office of OIG or the GSAM.
  • Must maintain active Medicare billing status.
  • Strong relationship-building skills; ability to influence without authority.
  • Decisive, collaborative, and skilled at gaining team buy-in.
  • Proven ability to drive clinical quality and productivity.
  • Comfortable serving as the clinical voice of the practice.
  • Tech-savvy with Google Suite and clinical platforms.
  • Able to multitask and thrive in fast-paced settings.
  • Strong customer service focus.

Preferred Qualifications

  • 1+ years of clinical leadership experience (ideally in virtual care).
  • Experience in value-based care (VBC) models.

Compensation

The United States new hire base salary target ranges for this full-time position are: $118,640 - $167,580 + bonus + equity + benefits

Starting base salary will be determined by job-related factors unique to each candidate, including but not limited to: education, training, skills, years and depth of experience, certifications and licensure, organizational needs, internal peer equity, and understanding of geographic and market data.
